New Patients

  • What do I have to bring on my first appointment?

    • Referral letter from GP or Specialist
    • XRays, MRI scans, ultrasound scans and all relevant reports
    • Medicare card, Health Fund card and Pension card
    • Bring a pair of shorts or wear loose trousers that can be rolled up.
  • Workcover

    If you are a workcover, you will need to bring your approval letter with a claim number. If this is not available, you will have to be out of pocket as per private patients.

  • How much does it cost?

    • Initial consultation is $260, if eligible Medicare rebate is $78
    • Subsequent consult is $150, if eligible Medicare rebate is $38

Patients going for surgery

  • I am going for surgery, what do I have to do?

    You will be given a surgical quote by the Dr Stephens’ office. If you are agreeable, you will need to sign the surgical quote which is also the financial consent form. You need to return this to the office at least 2 weeks or soonest prior to your surgery either by email (preferred method) or hand in to the office.

    You will have to settle your deposit at least one week prior to your surgery, to have your surgery booking confirm. The practice will endeavour to accommodate your requested dates and hospital of choice for surgery.

    You will need to complete the patient preadmission forms. The paper copy will be given to you at the time that you sign the surgical consent/ admission forms with Dr Stephens.

  • If you are going to have your surgery at North Shore Private Hospital:

    Please complete the paper copy and hand deliver to the hospital reception on the ground floor, main entrance ( Westbourne Street). Alternatively, you can complete the patient preadmission forms online, go to www.northshoreprivate.com.au. And click on online admission form or visit www.yourhealthportal.com.au. Please complete at your earliest convenience prior to admission.

  • If you are going to have your surgery at Castlecrag Hospital

    Please complete the paper copy and hand deliver to the hospital reception. Alternatively, you can complete the patient preadmissions online, go to castlecragprivate.com.au. Click on online admission form or visit www.mycare.ramsayhealth.com.au. Please complete at your earliest convenience prior to admission.
  • When do I go to the hospital, where do I go and what should I bring with me?

    You will be contacted by the hospital the day before surgery, and will be advised. Please do read the Admission Information Booklet (this is given to you when you sign the consent forms with Dr Stephens) for further details.

  • Unwell before surgey?

    If you are unwell the day before or the day of surgery, please advise Dr Stephens as soon as possible at 02 9157 1988 or email: reception@aortho.com.au
